You exercise regularly, you eat well, you feel fine. So why bother with a health check-up?
It's a question most of us ask ourselves, but here's the thing: some of the most serious health conditions show absolutely no symptoms until they become a crisis. And by then, the damage can already be done.
At Smart Salem, we see this reality play out every single day. In our Premium health screening programmes, we regularly uncover conditions that our clients had no idea existed. Two of those stories stand out as powerful reminders of why preventative health isn't a luxury; it's a necessity.

Case Study 1: James, 39, Finance
James (not his real name) was 39 years old, working long 12-14 hour days in finance and genuinely believed he was doing everything right. He exercised four times a week, mixing cardio and weights, took a stack of vitamins and minerals, had the occasional drink on weekends, and felt great.
He didn't come to Smart Salem because he was worried about his health. He came in for a routine visa medical. That's when everything changed.
What the screening found
James's Premium screening flagged something alarming: severe liver cell damage. His serum iron levels were at 411 which is more than double the normal upper limit of around 193. The culprit? The iron supplements he was taking daily, four tablets of 45mg, believing he was doing his body a favour.
"We see this all the time — people thinking they are making 'healthy' decisions but really, they are making things worse because they haven't invested in getting a baseline test done. In this case a simple blood test literally saved his life because he can now have treatment for his condition." — Dr Phillip Mirbach, Smart Salem
What happened next
James was immediately referred to a liver specialist. Without this screening, his condition would have been left completely undetected and untreated and could have led to a severe liver emergency and multi-organ failure. James changed his supplements entirely, adjusted his lifestyle based on what his body actually needed, and is now on the right path.
The lesson? Feeling healthy is not the same as being healthy. And doing something that seems healthy, like taking iron supplements, can actually cause serious harm if it's not based on your body's actual data.
Case Study 1: Nicole, 32, Management
Nicole (not her real name) was 32, recently promoted into a demanding management role, and had spent two months feeling utterly exhausted. She'd stopped working out. She'd stopped socialising. She assumed it was burnout because she'd just stepped up into a bigger job and the pressure was real.
She tried the classic approaches: eating more red meat in case it was low iron, cutting back on alcohol, and resting more but nothing helped. Eventually, she came to Smart Salem for a screening.
What the screening found
Nicole's results revealed a severe folate (Vitamin B9) deficiency, with her folate levels at just 3.55 which is well below the normal range of 4.8–37.3. She also had a minor Vitamin D deficiency. A gut microbiome test showed that while her gut health was generally good, her microbiome had a very low capacity to produce folate therefore meaning the problem wasn't just about what she was eating.
The treatment and recovery
Nicole was given targeted folate and Vitamin D supplements, along with a personalised diet plan designed around her specific gut microbiome gaps and not just a generic "eat more leafy greens" recommendation.
Within a single month, her burnout had significantly improved and her energy came back. She was exercising again, socialising again, performing at work again. The results were confirmed clinically too: her folate levels rose from 3.55 to 36.3 within two months, and remained above 20 nine months later.
The bigger picture
Nicole's story is incredibly common. Burnout and nutritional deficiencies share so many symptoms, fatigue, low mood, disrupted sleep, poor concentration, that it's almost impossible to tell them apart without testing. And yet, in our YouGov survey of over 1,000 UAE residents, 42% said they'd had a health check in the past year, while 23% had never had one at all.
So, What Is Your Body Not Telling You?
Both James and Nicole are real people who came to Smart Salem for entirely different reasons. One was there for a visa medical. One was desperate to find out why she felt so awful. Neither expected to uncover something that would change their health trajectory so dramatically.
